[目的]探讨胃三针结合健胃舒颗粒治疗脾虚气滞型功能性消化不良的临床疗效。[方法]将43例患者按随机数字表法分为治疗组22例和对照组21例。治疗组采用针刺中脘、足三里、内关,并口服健胃舒颗粒;对照组予枸橼酸莫沙必利口服。共治疗4周,观察和比较两组症状积分、疗效指数、水负荷试验和生活质量(SF-36)得分的差异变化。[结果]治疗组的总有效率为86.36%,与对照组比较差异有统计学意义(P0.05);治疗后两组的症状积分均有下降,水负荷试验得分、生活质量得分均升高,组间比较,治疗组改善更明显,差异有统计学意义(P0.05)。[结论]胃三针结合健胃舒颗粒治疗脾虚气滞型FD症状改善明显,疗效确切,值得临床广泛推广。  相似文献

目的:探讨崔书克教授对瘿病的病因病机的认识和治疗经验,为临床治疗提供经验和参考。方法:通过门诊跟师,整理崔教授多年来诊治本病的典型医案,通过研究分析其辨证思维和用药规律,试总结相关治疗经验,将理论与实践相结合,辅以具体验案加以佐证。结果:崔师擅用经方,结合多年临床经验,认为本病的治疗多从肝脾论治,运用小柴胡汤和当归芍药散加减治疗瘿病,不仅能够改善临床症状,而且能够既病防变,有效减缓该病的进一步发展。结论:崔教授治疗瘿病经验丰富且疗效显著,诊治特色鲜明,临证方法独特,对诊治该病具有指导意义。  相似文献

Objective To explore the mechanism of Pi(Spleen)-deficiency-induced functional diarrhea(FD)model rats treated by Shenling Baizhu Powder(参苓白术散,SBP).Methods Thirty male Sprague-Dawley rats were randomly divided into 5 groups including control,model,low-,medium-,and high-dose SBP groups(SBPLDG,SBPMDG,SBPHDG),6 rats in each group,respectively.Pi-deficiency-induced FD rats model was developed through Radix et Rhizoma Rhei gavage for 7 days.After modeling,the rats were treated with 3 doses of SBP[0.93,1.86,and 3.72 g/(kg·d)],and the rats in the control and model groups were given pure water for 7 days.The diarrhea index was calculated.On the 7th and 14th days,the traveled distance of rat was measured by the open field test.Serum D-xylose content was determined by the phloroglucinol method and interleukin(IL)-10 and IL-17 levels were measured using an enzyme-linked immunosorbent assay kit.The content of Treg cells was determined by flow cytometry.Results Compared with the control group,the diarrhea index and IL-17 level in the model group were significantly higher and the total exercise distance and D-xylose content significantly decreased(P>0.05).The expression of IL-10 in the SBPHDG group was significantly up-regulated,and serum D-xylose level and Treg cells increased significantly compared with the model group(P>0.05).Conclusion High-dose SBP exhibited ameliorating effects against Pi-deficiency induced FD,which might be attributed to its modulations on intestinal absorption function as well as adaptive immunity in mesenteric lymph nodes of rat.  相似文献

BackgroundThe incidence of idiopathic membranous nephropathy (IMN) has recently increased remarkably. Immune dysfunction caused by disordered intestinal flora might be an important factor affecting IMN. The Jian Pi Qu Shi Formula (JPQSF) shows promise in treating IMN. Here, we sequenced 16S rRNA genes to compare intestinal flora between patients with IMN and healthy persons. We also conducted a randomized controlled clinical trial to further compare the intestinal flora of patients with IMN treated with traditional Chinese medicine (TCM) and western medicine (WM).MethodsAmong 40 patients with IMN treated at Department of Nephrology in Xiyuan Hospital, Chinese Academy of Traditional Chinese Medicine between July 2016 and December 2018, we compared 30 of them with 10 healthy persons (controls). The IMN group was randomly assigned to receive JPQSF (TCM) or immunosuppressant WM therapy in (n = 15 per group) for 6 months. Intestinal microbiota diversity was analyzed using alpha diversity and beta diversity. Intestinal flora that significantly differed between the groups was analyzed using MetaStat. The effects and safety of the therapies were determined based on the values for plasma albumin, 24-h urine protein excretion, serum creatinine, urea nitrogen, estimate glomerular filtration rate (eGFR), complete blood count, and liver enzymes. All data were statistically analyzed using Statistical Package for the Social Sciences (SPSS) 20.0 statistical software.ResultsBaseline characteristics did not significantly differ between the IMN and healthy groups, or the TCM and WM groups. After six months of treatment, 24-h urinary protein significantly declined in the TCM and WM groups (before and after treatment: 3.24 ± 1.74 vs. 1.73 ± 1.85 g, P < 0.05 and 3.94 ± 1.05 vs. 1.91 ± 1.18 g, P < 0.05, respectively). Plasma albumin was significantly increased in the TCM group (before vs. after treatment: 32.44 ± 9.04 vs. 39.99 ± 7.03 g/L, P < 0.05), but did not significantly change in the WM group (31.55 ± 4.23 vs. 34.83 ± 9.14 g/L, P > 0.05). Values for urea nitrogen, serum creatinine, and eGFR did not significantly change in either group. The alpha diversity index for intestinal flora differed between the IMN and healthy groups, and the TCM and WM groups. Comparisons of multiple samples (beta diversity) revealed differences in intestinal flora between the IMN and healthy groups, and the TCM and WM groups. The Metastat analysis findings showed that the main genera that differed between the IMN group before treatment and the healthy group were Christensenellaceae_R-7_group, Bifidobacterium (77), Dorea, Escherichia-Shigella, Parabacteroides, Bifidobacterium, and Coprococcus_3. After TCM therapy, the main differential genera were Butyricimonas, Bacteroides, Alistipes, and Lachnospira, and after WM therapy, these were Ruminococcus_2, Lachnospiraceae_ND3007_group, Lachnospira, Bifidobacterium, Alistipes, and [Eubacterium]_ventriosum_group.ConclusionPatients with IMN might have disordered intestinal flora, and JPQSF can regulate intestinal flora in patients with IMN.  相似文献

目的探讨美皮康敷料在ICU压疮高风险患者皮肤保护中的有效性。方法选取2015年1-6月入住我科的压疮高风险的1 030例患者作为干预组,对患者预防性地使用美皮康敷料;2014年7-12月入住我科未进行敷料预防性保护的压疮高风险974例患者作为对照组。比较两组压疮的发生情况。结果干预组与对照组相比,压疮的发生率从3.49%下降到1.94%,差异具有统计学意义(P=0.032);干预组出现压疮的风险比为0.409[95%CI(0.211,0.793)]。结论美皮康敷料能够有效预防ICU压疮高风险患者压疮的发生。  相似文献

The pathological changes of parasympathetic nerve are considered as an independent prognostic factor of the survival rate of patients with chronic liver disease. The non-selective muscarinic acetylcholine receptors (mAchR) agonists and antagonists can affect the proliferation of hepatocytes and hepatic stellate cells, but the subtypes of mAchR expressions in HCs are still uncertain. Here, we investigate the expression of mAchR in hepatic fibrosis on rats. 3 ml/kg 40% carbon tetrachloride (CCL4) was given to induce hepatic fibrosis on rats and the hepatocytes were isolated. Compared to the normal state, the expression levels of m1, 3, 5 in fibrotic liver tissues or hepatocytes were obviously increased, while m2, 4 decreased. 10 μM pilocarpine or 10 μM acetylcholine could increase the alanine aminotransferase (ALT), hydroxyproline (Hyp), collagen I, III in the hepatocytes, and decreased albumin (ALB). They also changed the expressions of mAchR similarly as the fibrotic hepatocytes and livers. However, atropine could ameliorate the state of fibrotic hepatocytes. These data indicate that mAchR played an important role in the regulation of hepatic fibrosis process. Targeting mAchR would have therapeutic potential for hepatic fibrosis.  相似文献

[目的]观察杨振国术柏痛风汤治疗痛风疗效。[方法]将52例痛风使用术柏痛风汤,1剂/d,早晚各1次,水煎服。15d为1疗程。治疗3个疗程进行疗效判定。[结果]临床治愈32例,显效11例,有效5例,无效4例,总有效率为92.31%。[结论]术柏痛风汤治疗痛风疗效满意,值得推广。  相似文献

目的:观察四妙止痛汤配合熏洗治疗急性痛风性关节炎的临床疗效。方法:将60例急性痛风性关节炎患者随机分为2组。治疗组30例给予四妙止痛汤配合熏洗治疗,对照组30例给依托考昔片(安泰信)口服,疗程均为10天。结果:治疗组临床愈显率为96.7%,对照组愈显率为83.3%。两组关节疼痛、肿胀在治疗第5天和第10天后与治疗前比较均有非常显著性差异(P〈0.01);两组治疗后WBC较治疗前降低(P〈0.05),BUA、ESR均较治疗前明显降低(P〈0.01);治疗组的BUA下降较对照组明显(P〈0.05)。结论:四妙止痛汤配合熏洗治疗急性痛风性关节炎有较好的止痛消肿疗效,并能降低血尿酸。  相似文献
